The Supported Attachment Centre is an optional service that is aimed at improving the experience of both clinicians and patients using Health Care Connect.
Patients will be referred from a variety of sources, but the majority will be through Health Care Connect.

Yes, you may opt to continue to receive patients from Health Care Connect without any additional service.

Lakehead Nurse Practitioner-Led Clinic (NPLC) leads this project.
Note: people receiving supported attachment are not patients of Lakehead NPLC and no services will be billed to OHIP.

We can tailor the delivery to your clinic’s preferences. Information can be faxed, sent through an Ocean form, or we can facilitate agreements to directly enter data into your EMR.

Complexity matching is a process for assessing the level of support a patient will need and the drivers of their complexity.
Patients can then be matched to clinics that have capacity to support their needs, improving both patient experience and care team wellbeing.

Anyone who is unattached can sign themselves up to Health Care Connect. If there is a specific concern that they need addresssed, you can fax the clinics at (807) 475-9797 with ATTN: Supported Attachment Centre.
Examples of reasons to fax include:
Positive cancer screen
Suspicion of COPD or Heart Failure
Require gender affirming care
Would benefit from care in a language other than English
They do not have an Ontario Health Card
Any other reason you can identify
