Education For: Providers practicing in a clinic within the Noojmawing Sookatagaing OHT catchment area who are interested in becoming Certified Respiratory Educators (CRE) or want to learn how to administer and interpret Spirometry tests.

Equipment For: Clinics within the Noojmawing Sookatagaing OHT catchment area with a CRE (or staff member in CRE training) who want to offer spirometry but need assistance to purchase a spirometer and accessories.

COPD Care Map

For: Providers whose patients present with COPD or symptoms of COPD.

Services Offered: The care map is a resource to support providers to navigate their patients with COPD to local resources.

The care map includes: diagram overview of COPD diagnosis, provider resources, patient resources, and referral information by local health hub.

Best Care

For: Clinics looking to offer a spirometry program but are experiencing barriers (e.g., lack of staffing).

Services Offered: In-Clinic support to run a spirometry program on certain days (to be determined between participating clinic and Best Care).

Community-Based Pulmonary Rehab Education Program Toolkit

For: Providers or clinics looking to offer a community-based pulmonary rehab education program in their local community.

Toolkit offers: A full 9-week education program including all required resources.

Remote Care Monitoring

For: Providers treating COPD patients.

Services Offered: Referral information for high complexity and low complexity COPD cases, as well as those experiencing mobility challenges in the City of Thunder Bay.
