The Heart Failure Care Map is a resource to support providers in navigating individuals with heart failure to local resources.

How to use Care Map:

1. Review Heart Failure Primary Care Pathway. At each step, click on the note symbol for expanded information.

2. To find referral information, scroll to the Service Menu section of this page and select the Local Health Hub your patient/client lives in. Follow referral instructions as listed.

Additionally, we have a list of resources including:

• Provider Resources: A small, curated list of relevant clinician-facing resources

• Patient Resources: Includes handouts, educational videos, and programs to which patients can self-refer.
