Ontario Health invites you to join a regional implementation session for the North West Primary Care Capacity Plan.
Who: Family Medicine Specialists, Primary Care Nurse Practitioners, Ontario Health Teams, Primary Care Networks, Indigenous partners, patient and family advisors, and regional collaborators.
What: An interactive working session focused on advancing implementation of the North West Primary Care Plan.
When: September 17, 2026, 12:00 - 5:00 PM
Where: Superior Inn
Why: Actively co-designing the future state of primary care in the region, validate implementation priorities, and inform practical solutions that support physicians, teams, and patients.
During the session, participants will:
Receive an update on progress made since the regional planning and engagement process
Contribute to the active co-design of implementation approaches across the four planning pillars
Advance discussions and confirm approaches to digital governance in partnership with Indigenous communities and organizations
Explore opportunities for a shared supports model
Strengthen regional alignment and collaboration
