Care Map Key Features:

• Diagram: Provides an overview of Heart Failure diagnosis and management, including information to assist clinicians to determine when to make a referral

• Provider Resources: A small, curated list of relevant clinician-facing resources

• Patient Resources: Includes handouts, educational videos, and programs to which patients can self-refer

• Referral by Local Health Hub: Inventory of relevant programs, broken down by Local Health Hub.
